Hospice Services Hospice of The Gorge can help you to retain your personal dignity, achieve closure and make important decisions regarding your end-of-life goals. We collaborate with your personal physician and draw from a team that includes doctors and nurse practitioners, nurses, home health aides, social workers, chaplains, homemakers and volunteers to design a care plan that honors your unique needs and values. We can help whether you are in your own home, or in an assisted-living or other setting. Hospice care is about how you live—even with a life-limiting illness, there are choices. Hospice of The Gorge's medical team (including your personal physician) works to manage your physical symptoms so you can continue to be who you are and do what you do. We can deliver medications, medical equipment and supplies to your home to enhance your comfort and safety. We can provide counseling and grief support, and practical end-of-life resources such as creating a personal advance directive. We can send someone to do light housekeeping, or visit with you while your caregiver takes some personal time. • Who is eligible? |
