How are these services accessed? Anyone can "refer" a patient to Hospice of The Gorge: a patient or family member; a physician or other provider; or a care facility. A Hospice nurse then makes an appointment with the patient and family members to present our services and answer any questions. Once you decide to access Hospice services, we take care of the details. We work with your physician—who remains an integral member of the Hospice care team—to coordinate a care plan. We file the paperwork and arrange to have the necessary medicines, medical equipment and supplies delivered to your home.  |
