Process for Admission to Hospice

Coping with the many changes in life related to a life-limiting condtion can be bewildering to say the least. At times it can be debilitating for family members involved.
That's why we do everything in our power to make your transfer to Hospice of The Gorge care as simple and easy as possible.
We Make All the Arrangements for You
No one wants to worry about paperwork and insurance forms at a time like this. That's why we take most of this sort of hassle out of your way.
We do the paperwork for you, we make the arrangements with your various other care providers, we get equipment and medicines delivered to your home ... whatever it takes, our job is to help you begin to feel better, and to live the highest quality of life possible, for as long as possible.

What You Will Need To Do
First we encourage you to consult with your family physician to see if hospice is right for you. In addition, we welcome your call to find out if Hospice of The Gorge would be the right hospice to meet your needs.
Once you choose Hospice of The Gorge, we take most of the work off of your shoulders:
- We will work directly with your physician to secure your admission orders, medical records and other information that may be helpful.
- All you will really need to do is sign the appropriate admission documents, and make certain types of decisions about the care you want.
- The types of decisions you will need to make include things such as: Would you would want a hospice chaplain to come visit; would you want someone to help you with household chores; and who is authorized to speak on your beh
alf or on behalf of your loved one.

If You're the Family Member ...
Above all, expect courteous, respectful, prompt and professional support throughout the admissions process. Know that everyone working on your hospice team cares. We want to make the process as easy as it can possibly be for you.
If You're the Physician or Discharge Nurse ...
Our goal is to make things as easy as possible fo you, too. We know the many challenges and pressures you're faced with, so we do whatever we can on our end to simplify things for you.
We are also happy to step you through the process of what the Medicare requirements are as well as requirements of any state and federal entities that may oversee hospice care.
